TRANSITIONS

The "Transitions" program, here at Martin House, offers an opportunity for residents in the "Recovery for Life Program" to successfully practice greater independence without having to move to another facility. With this intensive support, the Residential Support Staff can ensure necessary linkages are in place for continued recovery. Supports include:

 

  • Assistance with transportation linking to provide services for housing.

  • Coordinating with family or service providers regarding entitlements with conservators, DSS, SSDI and SSA office.

  • Employment services are provided to outside resources to assist with job security within the

  • community.

  • Education within Martin House to collaborate with the resident for all outside supports.

  • Financial literacy which supports and encourages maintenance with the residents’ means.

 

This ultimately includes but not limited to housing authorities, boarding care or independent
living in the community.

 

The goal is to provide supports for one year to two years based on residents’ readiness to
succeed in a less intensive housing support program.
